		Washington State Adult Soccer Association The WSASA was formed to provide the widest opportunity for all adults to participate in the game of soccer while maintaining a safe playing environment within the laws of the game.

		United States Adult Soccer Association USASA is a National Organization member of the United States Soccer Federation (USSF), and is currently the only adult soccer member organization of USSF.

Supplemental Insurance from USASA

If you get injured while participating in an ICWSL-sanctioned event (e.g., a scheduled league game), you may be eligible for supplemental insurance from the United States Adult Soccer Association (USASA).

This coverage is EXCESS of other insurance. Please be sure to submit other insurance information (if available) when requested.

The claim form below is designed specifically for USASA. Please use only this form. Do not delay submitting this form. The form must be received, with or without attachments, within 90 days from the date of the accident or benefits may be denied due to untimely filing. Submit completed insurance forms to Washington State Adult Soccer Association for verification. The state office will submit verified insurance claims to USASA on your behalf.
